3/9/2024 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Jeremiah appeared to be in a positive mood evident by his facial expressions such as a smile on the face. QMHP-C informed Jeremiah this session was going to continue to focus on his behaviors reported by his mother as well as self-report and working towards his ISP goals. Mother reports when he is upset he will lash out on others at least 3 times weekly with behaviors lasting up to 10 minutes in duration. 2. Behavior: Jeremiah appeared to be non-complaint throughout the session and mother was providing QMHP-C with an updates on his behaviors throughout the session at which behaviors last up to 2 minutes in duration. 1.Intervention: Previous session and positive behaviors displayed throughout the day were explored. Medication compliance was conducted with guardian. QMHP-C engaged him in conversation on improving his level of self- control. QMHP-C inquired on any changes in mood negative interactions with others. QMHP-C discussed ways that he has been working on his self-control over the past few weeks. QMHP-C asked him questions to determine the source of his issues with self- control. 2. Intervention: QMHP-C reviewed with him things that he can do daily to work on controlling and acting actions. QMHP-C discussed ways to change things about his behaviors that he is not happy with. QMHP-C discussed the concept of positive self-talk that he has utilized in previous sessions. QMHP-C discussed the effectiveness of positive self-talk. QMHP-C reminded him that he should engage with others more in order to work on improving his level of self- control. QMHP-C reviewed positives characteristics about himself and how to focus on those daily. 1.Response: He reported taking his medication and denied any side effects. He was receptive to ways to improve his self- control. He reports that he has low self- control at times. 2. Response: He stated that he uses positive self-talk sometimes but not on a daily basis. He agreed to begin to work on trying to engage with others more and change some things about himself in an effort to address his low level of self- control.
